HOTEL RESERVATION FORM
Hotel accommodations are available at the Kona Surf Resort and Country Club. The sleeping rooms at the Kona Surf are spacious and most have breath-taking views of the ocean while others have views of historic Keauhou Bay. Built on varied levels, the Resort has three wings extending on dramatic cliffs to the ocean front, with a six-story tower set back in an exotic garden setting.

Sleeping rooms are air-conditioned room, have color TV (selected cable stations and pay movies available), telephones, safes for valuables, mini-refrigerators, separate dressing areas and balconies. Rooms for non-smokers and the physically challenged are also available.

Reservations should be made directly to the hotel. Please make your hotel reservations as soon as possible to ensure availability at the conference rates. Mail, call, fax, or e-mail the hotel directly (information listed below), and mention that you are a participant in the AGU Chapman Conference on Magnetospheric Current Systems to be held January 11-15, 1999.

Rates are per room, per night. Room rates are subject to a 4.16% Hawaii State Tax and a 7.25% Hawaii Hotel Tax (effective January 1, 1999). A one night deposit plus hotel tax is required no later than December 10, 1998 (major credit cards, money orders, and checks are accepted).

Conference rates will apply from January 7, 1999 through January 18, 1999. Room requests received after December 10, 1998 or for additional nights prior to January 11 or after January 15, 1999 will be subject to availability.

A maximum of 4 persons per room are allowed, including children. There is an additional charge of $20 per person for the third and fourth persons in a room. Children under the age of 18 can stay in the room free of charge when sharing a room with parents and using existing bedding.

72 HOURS CANCELLATION NOTICE PRIOR TO YOUR ARRIVAL IS REQUIRED TO AVOID FORFEITURE OF DEPOSIT.
